  • 1. A copy of your valid Resale/Business Certificate
  • 2. Or, download and complete the Universal Resale Certificate
  • 3. A copy of your applicable state(s) business license
  • 4. A copy of your purchasers valid State Identification
  • 5. If you company is based in the State of New Jersey and is tax exempt, provide a copy of your tax exemption certificate.

Upon receipt and approval of this information we will contact you to discuss your specific needs and then send your first Kooli Order Form.
